Tranexamic acid (TXA) has been identified as a key therapeutic player in the management of postoperative bleeding and transfusion requirements. The aim of this study was to assess the potential benefits of the IV administration of TXA as a means to reduce blood loss in patients undergoing total knee arthroplasty. This prospective, randomized, double-blind, placebo-controlled study was conducted at 13 hospitals in China from May 2015 to January 2016. The primary endpoint was reduction in blood loss, and ancillary outcomes included the use of blood products and the incidence of thromboembolic and other adverse events. A total of 283 patients were randomly assigned to the TXA or placebo group (143 patients in each group). No difference in blood loss between the TXA and placebo groups was observed at 4 hours postoperatively (mean difference, 64.71 mL [95% confidence interval {CI}, -16.18 to 146.61 mL]). However, by 48 hours postoperatively, significantly more patients in the TXA group required blood transfusion (odds ratio, 2.30; 95% CI, 1.21-4.37). Similarly, thromboembolic adverse events were more frequent in the TXA group at 48 hours postoperatively (odds ratio, 1.89; 95% CI, 1.00-3.59).
